What does the AZ modifier mean?
The HCPCS Level II modifier AZ means: “Physician providing a service in a dental health professional shortage area for the purpose of an electronic health record incentive payment”. Append it to a HCPCS or CPT code on the claim line when the payer’s policy calls for it; placement and payment effects are defined by each payer.
